Temporary License Application

If you are planning to visit Saskatchewan and wish to obtain a temporary license please fill out the APPLICATION.

Please Submit:

1. proof of good standing from current licensing body
2. a copy of your current licence certificate (if applicable)
3. proof of $5 million Malpractice Insurance
4. fee of $50.00 if attending/instructing an educational course that involves treating clients AND is of two weeks duration of less. Courses over 15 days duration must apply for a practicing licence.
