If you are planning to visit Saskatchewan and wish to obtain a TEMPORARY LICENSE:
1) Print, fill-in and scan, fax or mail
2) Fill in the Word Form and email
Please Submit:
- Proof of good standing from current licensing body
- A copy of your current licence certificate (if applicable)
- Proof of $5 million Malpractice Insurance
- Fee of $50.00 if attending/instructing an educational course that involves treating clients AND is of two weeks duration of less. Courses over 15 days duration must apply for a practicing licence.
