Reinstatement Application
This process is for members that have previously been licensed with SCPT. Please login to your profile and complete the Reinstatement Application. If you do not have your login credentials, please contact the SCPT office at 306-931-6661 or operationsmanager@scpt.org.
Application Process
- Login to your member profile
- Complete the online Reinstatement Application Form (you will find the link to this form on your homepage when you login
- Provide proof of $5,000,000 Professional Liability Insurance
- Provide Letter of Good Standing from your current or most recent licensing body or have the Regulatory History Form completed
- Provide a copy of your current license.
- Provide a completed current Criminal Record Check, including a Vulnerable Sector check.
- Complete the jurisprudence questions
- Pay the required fees (subject to change without notice):
- $500 Initial registration and Membership fees ($250 after October 1)
- $75 non-refundable initial registration fee
- Total = $575 CAD ($325 after October 1)
